Job Description:   Essential Duties: + Evaluate and manage chronic heart failure patients at all stages of the disease process + Requests appropriate diagnostic studies and laboratory testing to aid in diagnosis and management of heart failure patients + Coordinate patient care between specialty physicians, nursing staff and other interdisciplinary providers + Prescribes and administers medications based on clinical need + Serves as a liaison for the physician in communicating with patients and their families + Initiates arrangements for hospital admissions + Provides and coordinates patient teaching and counseling + Office Schedule- Monday – Friday Qualifications Education: + Master's degree in Nursing. + Successful completion of an approved Nurse Practitioner program. Experience: + Two years of previous Nurse Practitioner experience preferred.
